目的探讨血清超敏C反应蛋白(hsCRP)、脂蛋白(a)[Lp(a)]及血脂水平评估糖尿病肾病(DN)肾损害。方法 186例2型DN患者根据24-h尿白蛋白排泄率分为B组(<30mg/24-h,68例)、C组(30-300mg/24-h,58例)和D组(>300mg/24-h,60例),选择88例健康体检者为对照(A)组,检测四组血清hsCRP、Lp(a)、TC、TG、LDL、HDL水平。结果 B、C、D组hsCRP、Lp(a)、TG均高于A组(P<0.05)。C组LDL高于A组而HDL低于A组(P<0.05)。D组TC、LDL高于A组而HDL低于A组(P<0.05)。D组hsCRP、Lp(a)、TG、TC、LDL水平均高于B组而HDL低于B组(P<0.05)。D组hsCRP、Lp(a)、TC、LDL水平高于C组(P<0.05)。B组hsCRP、TG低于C组而HDL高于C组(P<0.05)。结论 hsCRP、Lp(a)、血脂可作为诊断DN肾损害的敏感指标,联合检测更具实用价值。
Objective To investigate the effects of serum hsCRP, lipoprotein (a) [Lp (a)] and serum lipids on diabetic nephropathy (DN). Methods 186 patients with type 2 DN were divided into group B (n = 30), group C (30-300 mg / 24 h, n = 58) and group D according to the urinary albumin excretion rate (300mg / 24-h, 60 cases). Eighty-eight healthy subjects were selected as control group (A). Serum hsCRP, Lp (a), TC, TG, LDL and HDL levels were measured in the four groups. Results The hsCRP, Lp (a) and TG in groups B, C and D were higher than those in group A (P <0.05). LDL in group C was higher than that in group A and HDL was lower than that in group A (P <0.05). The TC and LDL in group D were higher than those in group A, while HDL was lower than group A (P <0.05). The levels of hsCRP, Lp (a), TG, TC and LDL in group D were higher than those in group B and HDL was lower than that in group B (P <0.05). The levels of hsCRP, Lp (a), TC and LDL in group D were higher than those in group C (P <0.05). Group B hsCRP, TG lower than the C group and HDL higher than the C group (P <0.05). Conclusions hsCRP, Lp (a), lipids can be used as sensitive indicators for diagnosis of DN nephropathy, combined detection of more practical value.
