患者,女性,24岁,主因妊娠7个月第一胎,无痛性阴道出血10天,未破水入院,查体:体温、脉搏、呼吸、血压均正常。产科情况:腹部如孕8个月大小,无压痛,无宫缩,宫高26cm,腹围89cm,胎位左枕横,胎心140次/分,耻上未闻及胎盘杂音,骨盆外测量正常。临床诊断:①宫内孕31周第一胎左枕横无产兆;②前置胎盘。 B型超声检查:胎头耻上,双顶径8.3cm,胎位左枕前,胎心搏动可见,羊水深4.3cm,胎盘后壁Ⅰ~+级,胎盘边缘部分覆盖于宫颈内口之上,左侧壁可见部分胎盘回声,B型超声提示:①宫内孕单活胎;②部分性前置胎盘;③副胎盘。
