1 为何说多器官功能衰竭是近年来引起重视的新的临床综合征?多器官功能衰竭(MOF)是指在严重感染、休克、创伤、烧伤、大手术及急性胰腺炎等应激状态下,心、脑、肺、肾、胃、肠、胰腺及血液等器官中,短期内有2个或2个以上器官相继或同时发生功能衰竭,不能自行维持其功能活动的综合征.由于免疫系统、凝血系统和代谢系统也可发生功能严重障碍,因此MOF又称为多系统器官功能衰竭(MSOF).多器官功能衰竭是本世纪70年代开始形成的概念.1973年Tilney等首次报道了18例主动脉瘤破裂术后并发急性肾衰的病人相继发生呼衰和败血症等重要器官衰竭;Bane(1975年)对此作出系统研究,并在一些尸检材料中观察到3例临床诊断为某一器官功能衰竭的病例均有多个器官的严重病变,发现多个器官衰竭的证据,提出了多发进行性或序贯性系统衰竭是一个“70年代的综合征”.Eiseman等1977年提出了多器官功能衰竭(MOF)这一医学术语.
