目的研究分析耳鸣患者合并睡眠障碍的临床特征。方法采用随机、对照的方法,选择50例耳鸣患者及50例无耳鸣主诉的对照人群,对耳鸣组患者的性别、年龄、病程、耳鸣侧别、起病情况、听力情况、全身疾病、伴随症状等进行调查,行耳内镜检查、听力评估及影像学检查,并使用耳鸣残疾评估量表(tinnitus handicap inventory,THI)评估耳鸣残疾水平,对全部研究对象使用匹兹堡睡眠质量指数量表(Pittsburgh sleep qua Iity index,PSQI)评估睡眠质量。结果 50例耳鸣患者之中,有睡眠障碍表现38例(72%),平均PQSI得分10.64±4.89分;对照组有睡眠障碍者16例(32%),平均PQSI得分5.32±5.33分,差异有统计学意义(P<0.01)。两组PQSI评分各项目除使用睡眠药物情况外,PQSI量表所涉及主观睡眠质量、睡眠潜伏期、睡眠持续性及习惯性睡眠效率、睡眠紊乱、白天功能紊乱6个指标差异均具有统计学意义(P<0.01)。对睡眠障碍相关危险因素分析发现,耳鸣的响度与睡眠障碍总分相关(P<0.01)。对于耳鸣严重程度相关危险因素分析提示,睡眠障碍障碍总分是加重耳鸣严重程度相关因素(P<0.01)。结论耳鸣患者易伴随睡眠障碍且二者相互影响,在耳鸣治疗中开展患者睡眠障碍治疗,改善睡眠质量同时利于耳鸣康复。
Objective To analyze the clinical features of tinnitus patients complicated with sleep disorders. Methods Fifty cases of tinnitus patients and 50 control subjects without tinnitus were selected by randomized and controlled methods. The factors of gender, age, course of disease, tinnitus, onset, hearing, general condition, concomitant symptoms , Tinnitus handicap inventory (THI), tinnitus handicap inventory (THI), and Pittsburgh Sleep Quality Index Scale (Pittsburgh Sleep Quality Index Scale) qua Iity index, PSQI) to assess sleep quality. Results Among the 50 patients with tinnitus, 38 (72%) had sleep disorders, with an average PQSI score of 10.64 ± 4.89. There were 16 (32%) sleep disorders in the control group with an average PQSI score of 5.32 ± 5.33, with a difference of Statistical significance (P <0.01). In addition to sleep medication, PQSI scale had significant differences in subjective sleep quality, sleep latency, sleep duration and habitual sleep efficiency, sleep disorders, and daytime dysfunction among the two groups. P <0.01). Analysis of the risk factors associated with sleep disorders found that the loudness of tinnitus correlated with the total score of sleep disorders (P <0.01). Analysis of the risk factors related to the severity of tinnitus suggests that the total score of sleep disorder is related to the severity of tinnitus (P <0.01). Conclusions Tinnitus patients are easily accompanied by sleep disorders and affect each other. Patients with tinnitus tinnitus can be treated with sleep disorders to improve the quality of sleep and benefit the recovery of tinnitus.
