目的研究亚低温治疗对重型颅脑损伤患者血浆内皮素-1的影响并探讨其临床意义。方法回顾性分析我科2014年2月至2016年2月收治的48例重型颅脑损伤患者的临床资料,随机分为观察组(24例)与对照组(24例)。对照组给予手术去骨瓣减压、降颅压、防治感染等常规治疗,观察组给予常规治疗结合亚低温治疗。对比两组患者治疗后血浆内皮素-1的变化,通过两组患者格拉斯哥预后评分(GOS)比较,评定疗效。结果两组患者治疗后血浆内皮素-1较治疗前降低(P<0.05),且治疗后观察组结果低于对照组(P<0.05);观察组患者治疗三个月后GOS评分高于对照组,差异有统计学意义(P<0.05)。结论亚低温治疗可显著降低重型颅脑损伤患者血浆内皮素-1水平,改善患者预后。
Objective To investigate the effect of mild hypothermia on plasma endothelin-1 in patients with severe craniocerebral injury and to explore its clinical significance. Methods The clinical data of 48 patients with severe traumatic brain injury admitted from February 2014 to February 2016 in our department were retrospectively analyzed. The patients were randomly divided into observation group (24 cases) and control group (24 cases). Control group was given surgical decompressive craniectomy, reducing intracranial pressure, prevention and treatment of infection and other conventional treatment, the observation group given conventional treatment combined with mild hypothermia. The change of plasma endothelin -1 was compared between the two groups. The Glasgow Outcome Scale (GOS) was compared between the two groups to evaluate the curative effect. Results After treatment, the plasma endothelin - 1 in both groups was lower than that before treatment (P <0.05), and the results in the observation group were lower than those in the control group after treatment (P <0.05). The GOS scores in the observation group were higher than those in the control group Group, the difference was statistically significant (P <0.05). Conclusion Mild hypothermia can significantly reduce the level of endothelin-1 in patients with severe traumatic brain injury and improve prognosis.
