患者女性,14岁.1987年9月29日入院.一年前出现消瘦、乏力、怕热、食欲增加及活动后心慌、胸闷、情绪急燥.一周前受凉后出现咽痛、腹泻,心慌、气促明显,当地医院抗炎5天无效,心慌、气促进行性加重,并出现夜间阵发性呼吸困难,食欲下降,尿量减少,精神差,懒言.否认风湿热及心慌、浮肿史.体检:T37℃,P138次/分,R34次/分,BP21.3/12kPa.神志清,半卧位,重病容.双眼球突出,甲状腺Ⅲ度肿大,可闻及血流杂音,颈静脉怒张.两肺闻及干湿啰音.心界左下扩大,心率138次/分,律齐,心尖区闻收缩
