目的探讨磁共振T1ρ成像(T1ρWI)在原发性肝癌诊断中的应用价值。方法前瞻性收集2014年1月至2014年6月期间于四川大学华西医院诊治的、经超声或CT检查确定肝脏占位并经病理学穿刺活检证实为原发性肝癌的9例患者进行磁共振扫描,包括常规T1WI平扫、T2WI平扫、T1WI增强扫描、T1ρWI平扫及T1ρWI增强扫描,比较T1ρWI平扫及T1ρWI增强扫描与其他序列的信号对比度,以探讨T1ρWI在原发性肝癌诊断中的应用价值。结果与T1ρWI平扫比较,T1WI平扫和T1WI增强扫描的信号对比度较低(t=3.532,P=0.008;t=3.666,P=0.006),而T2WI平扫信号对比度的差异无统计学意义(t=-1.448,P=0.186);与T1ρWI增强扫描比较,T1WI增强扫描的信号对比度较低(t=3.468,P=0.008)。结论 T1ρWI能较好地显示原发性肝癌病灶,有望成为新的对原发性肝癌诊断有利的磁共振扫描方式,且其是一种新的不使用造影剂的动脉成像方法。
Objective To investigate the value of T1ρWI in the diagnosis of primary liver cancer. Methods Nine prospective patients who were diagnosed as primary hepatic cancer by ultrasound or CT examination and confirmed by pathologic biopsy from January 2014 to June 2014 were collected from January 2014 to June 2014 in our hospital. T1WI scan, T1WI scan, T1WI scan and T1WIWI scan were performed to compare T1WI scan and T1WIW scan with other sequences in order to find out the significance of T1WI in diagnosis of primary liver cancer Value. Results The contrast of T1WI scan and T1WI enhanced scan was lower than that of T1WI scan (t = 3.532, P = 0.008; t = 3.666, P = 0.006), while the contrast of T2WI scan signal was not statistically significant t = -1.448, P = 0.186). Compared with T1ρWI enhanced contrast, T1WI contrast scan showed a lower contrast (t = 3.468, P = 0.008). Conclusion T1ρWI can better show the primary liver cancer lesions and is expected to become a new magnetic resonance scanning method for the diagnosis of primary liver cancer. It is a new arterial imaging method without contrast medium.
