为研究急性坏死性胰腺炎(ANP)早期白细胞粘附强弱和肿瘤坏死因子(TNF)的变化及其与肺损伤的关系,以及大承气汤能否减轻肺损伤的程度,将14 只家犬随机分成承气汤组(n= 5,ANP模型+ 大承气汤治疗),盐水组(n= 5,ANP模型+ 生理盐水治疗)和假手术组(n= 4)3 组。术后在流室系统下观察白细胞与内皮细胞的粘附强弱,并测定血清及支气管肺泡灌洗液(BALF)TNF活性和肺损伤程度。结果显示:实验犬ANP时白细胞粘附活性增强,外周血及BALF中TNF活性升高;承气汤组白细胞粘附活性低于盐水组(P< 0.05),血清及BALF的TNF活性低于盐水组(P< 0.01);承气汤组肺损伤程度轻于盐水组。结果提示:白细胞粘附和TNF的过度分泌参与ANP病理生理及肺损伤的发生,大承气汤能够减轻二者介导的肺损伤。
To investigate the relationship between the early leukocyte adhesion and tumor necrosis factor (TNF) changes and lung injury in patients with acute necrotizing pancreatitis (ANP), and whether Dachengqi Decoction can reduce the degree of lung injury, 14 homes will be studied. The dogs were randomly divided into Chengqi Decoction group (n = 5, ANP model + Dachengqi Decoction treatment), saline group (n = 5, ANP model + saline treatment) and sham operation group (n = 4) 3 groups. After operation, the adherence of white blood cells and endothelial cells was observed under the flow chamber system, and the serum TNF activity and lung injury degree in bronchoalveolar lavage fluid (BALF) were measured. The results showed that the leukocyte adhesion activity was enhanced in experimental dogs with ANP, and the activity of TNF in peripheral blood and BALF was increased. The adhesion activity of white blood cells in Chengqi Decoction group was lower than that in saline group (P<0.05). The TNF activity in serum and BALF was low. In the saline group (P < 0.01); Chengqi Decoction, the degree of lung injury is lighter than the saline group. The results suggest that leukocyte adhesion and excessive secretion of TNF are involved in the pathophysiology and lung injury of ANP. Dachengqi Decoction can reduce the lung injury mediated by both drugs.
