目的探讨高压氧与尼莫地平联合治疗颅脑损伤所致脑功能障碍的临床疗效。方法选取神经外科2011年2月-2014年2月期间收治的因颅脑损伤所致的脑功能障碍患者124例,根据治疗方法不同将其分为常规治疗组45例、高压氧疗组40例和高压氧联合尼莫地平组(联合治疗组)39例。比较各组患者在治疗前、治疗后2周、治疗后4周的神经功能缺损评分以及脑血流量情况。结果治疗前3组神经功能缺损评分以及脑血流量无统计学差异(P>0.05)。治疗2周和4周后,高压氧疗组、联合治疗组患者的神经功能缺损评分低于常规治疗组,差异有统计学意义(P<0.05),而且联合治疗组患者的评分低于高压氧疗组,差异有统计学意义(P<0.05);高压氧疗组、联合治疗组患者的脑血流量高于常规治疗组,差异有统计学意义(P<0.05);联合治疗组的神经功能评分低于高压氧疗组,差异有统计学意义(P<0.05);联合治疗组的脑血流量高于高压氧疗组,差异有统计学意义(P<0.05)。结论高压氧与尼莫地平联合治疗能显著改善颅脑损伤所致脑功能障碍的神经功能缺损评分及脑血流量,促进患者神经功能的恢复。
Objective To investigate the clinical effects of hyperbaric oxygen combined with nimodipine on brain dysfunction induced by craniocerebral injury. Methods A total of 124 patients with brain dysfunction caused by craniocerebral injury were selected from January 2011 to February 2014 in Neurosurgery. According to the different treatment methods, they were divided into routine treatment group (n = 45), hyperbaric oxygen therapy group (n = 40) And hyperbaric oxygen combined with nimodipine group (combined treatment group) 39 cases. The neurological impairment scores and cerebral blood flow were compared before treatment, 2 weeks after treatment, 4 weeks after treatment. Results There was no significant difference in neurological deficit score and cerebral blood flow between the three groups before treatment (P> 0.05). After 2 weeks and 4 weeks of treatment, the scores of neurological deficits in hyperbaric oxygen therapy group and combination therapy group were lower than those in conventional therapy group (P <0.05), and the scores in combination therapy group were lower than those in hyperbaric oxygen therapy group (P <0.05). The cerebral blood flow of hyperbaric oxygen therapy group and combination therapy group was higher than that of conventional therapy group (P <0.05), and the neurological function of combination therapy group The score was lower than that of hyperbaric oxygen therapy group (P <0.05). The cerebral blood flow of the combination therapy group was higher than that of hyperbaric oxygen therapy group, the difference was statistically significant (P <0.05). Conclusion Combined treatment of hyperbaric oxygen and nimodipine can significantly improve neurological deficit scores and cerebral blood flow of brain dysfunction caused by brain injury and promote the recovery of neurological function.
