本文采用经颅多普勒超声仪(TCD)检测12例临床诊断脑死亡患者的大脑中动脉血流速度(Vmca)及频谱表现。发现Vmca介于一3cm/s~16cm/s,平均5.8cm/s。频谱变化表现为“震荡波”形(11支MCA),收缩期尖峰信号(10支),另3支未探及。上述表现与正常及重度颅脑外伤均有明显不同。12例患者均告死亡。我们认为:TCD在脑死亡诊断中准确性高,是一种有用的辅助手段。
In this paper, transcranial Doppler ultrasound (TCD) was used to detect the cerebral blood flow velocities (Vmca) and spectral features in 12 patients with brain-dead. Vmca was found to range from 3 cm / s to 16 cm / s with an average of 5.8 cm / s. Spectrum changes showed “shock wave” shape (11 MCA), systolic spikes (10), the other three were not explored. The above performance and normal and severe traumatic brain injury are significantly different. All 12 patients died. We think TCD is a useful adjunct to the high accuracy of brain death diagnoses.
