单核细胞性白血病可分二种类型:单纯的单核细胞性白血病(或称Schilling氏型)和混合的粒-单细胞性白血病(或称Naegeli氏型)。目前,尽管有特殊染色法,用光学显微镜区别这二型有时尚有困难。事实上,单纯的单核细胞型白血病的真正发病率比一般报导的要高得多。由于原始单核细胞内偶可含有较大的颗粒(电子显微镜见到其大小约800毫微米),致光学显微镜误诊为原始粒细胞。作者根据7例所见,归纳原始单核细胞与原始粒细胞的超微结构特征如下表。
There are two types of monocytic leukemia: simple myelomonocytic leukemia (or Schilling’s type) and mixed granulocyte-monocytic leukemia (or Naegeli’s type). At present, despite the special staining method, it is sometimes difficult to distinguish these two types by light microscopy. In fact, the true incidence of pure monocytic leukemia is much higher than generally reported. Because the original mononuclear cells can even contain larger particles (electron microscopy to see the size of about 800 nm), the optical microscope misdiagnosed as primitive granulocytes. Based on the findings of 7 cases, the author summarizes the ultrastructural features of primitive monocytes and primitive granulocytes in the following table.
