目的:探讨超声筛查胎儿肢体畸形的方法,了解胎儿肢体畸形的特点。方法:采用二维连续顺序追踪超声扫查法检测24 350例孕妇的胎儿肢体,规范化存储胎儿肢体标准切面图,发现胎儿肢体畸形或畸形不明显而形态或姿势异常者行三维超声检查,将产前超声诊断结果与引产或产后结果对照。结果:产后发现胎儿肢体畸形42例,产前超声检出33例,诊断符合率78.57%,漏诊9例,漏诊率21.43%。超声诊断畸形顺位为:多指(趾)畸形,足内翻,短肢畸形、手或足姿势异常、手缺如、足外翻。产后发现漏诊最多的畸形为多指(趾)畸形。结论:二维连续顺序追踪超声扫查法是筛查胎儿肢体畸形的有效方法,联合三维超声检查可提高胎儿肢体畸形的诊断准确率。
Objective: To investigate the method of ultrasound screening fetus limb deformity and to understand the characteristics of fetus limb deformity. Methods: Fetal limbs of 24 350 pregnant women were detected by two-dimensional sequential sequential sonographic scanning. The standardized sectional views of fetal limbs were normalized. Three-dimensional ultrasonography was performed on patients with abnormal or abnormal fetal limbs and abnormal fetal shape or posture. Ultrasound diagnosis before the results of induction or postpartum control. Results: In the postpartum period, 42 cases of fetal limb deformity were found and 33 cases were detected by prenatal ultrasound. The coincidence rate was 78.57%, misdiagnosis was 9 and the rate of misdiagnosis was 21.43%. Ultrasound diagnosis of malformations as follows: multiple finger (toe) deformity, varus, short limb deformity, hand or foot posture abnormalities, such as hand, foot valgus. Misdiagnosis of the most postpartum deformity found for multiple finger (toe) deformity. Conclusion: Two-dimensional continuous sequential ultrasound scanning is an effective method to screen fetal limb deformity. Combined with three-dimensional ultrasonography can improve the diagnostic accuracy of fetal limb deformity.
