目的通过调查分析湖北省内首例人感染H5N6禽流感患者的实验室检查结果,了解新发疾病H5N6禽流感的实验室检查特点,为H5N6禽流感提供更好的诊治依据。方法采集确诊患者的血液、体液标本,检测各项实验室检查指标。结果本省首例感染H5N6患者白细胞与中性粒细胞总数升高而红细胞减少;淋巴细胞呈现波动趋势;丙氨酸氨基转移酶(ALT)、天冬氨酸转氨酶(AST)在病毒核酸转阴过程中由明显增高后逐步恢复正常;肾功能、血脂糖等指标无明显异常;动脉血二氧化碳分压(pCO_2)持续增高,氧饱和度由低值转为正常;降钙素原(PCT)含量正常;辅助性T细胞(CD3~+CD4~+)降低,抑制性T细胞(CD3~+CD8~+)升高,CD4~+/CD8~+倒置;心肌肌钙蛋白I(cTnI)与N-末端脑利钠肽(NT-proBNP)含量升高。结论 2016年全球第7例湖北省首例H5N6禽流感患者临床表现较重、病程较长,掌握H5N6禽流感实验室检查的特点可为其诊治提供有效的参考依据。
Objective To investigate the laboratory test results of the first case of H5N6 bird flu infection in Hubei Province and to understand the characteristics of laboratory tests of the new-onset H5N6 bird flu so as to provide better diagnosis and treatment basis for H5N6 bird flu. Methods Blood samples and body fluid samples from patients were collected and tested for laboratory tests. Results The total number of white blood cells and neutrophils in the first H5N6 infection in our province was increased while the number of erythrocytes was decreased. The lymphocytes showed a trend of fluctuation. The activities of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) The levels of POC_2 and oxygen saturation increased from low to normal, and the content of procalcitonin (PCT) was normal (CD3 ~ + CD4 ~ +), elevated CD3 ~ + CD8 ~ +, CD4 ~ + / CD8 ~ + upregulation, cTnI and N- Elevated brain natriuretic peptide (NT-proBNP) levels. Conclusion In 2016, the 7th case of H5N6 bird flu in Hubei province had severe clinical manifestations and a long course of disease. To master the characteristics of H5N6 bird flu laboratory tests may provide an effective reference for diagnosis and treatment of H5N6 bird flu.
