目的:探讨64层螺旋CT仿真内窥镜(CTVE)及其它后处理技术对肠道病变的诊断价值。方法:对75例临床怀疑肠道病变的病人进行了空气充盈法64层CT增强扫描,在ADW4.4工作站进行重建,包括CTVE、MPR、VR、VD、MIP像。以手术、病理或随访结果作为参考标准,分析64层CT对肠道病变的诊断价值。结果:75例受检病人中,经临床综合诊断49例为器质性病变;64层CT检出病变45例,漏诊5例,假阳性1例,敏感度89.9%,特异度96.2%,阳性预测值97.8%,阴性预测值83.3%,准确性92%。CTVE对于黏膜面的细微病变及形态改变显示良好,但需与其他后处理方法相互结合才能做出正确诊断。结论:64层CTVE及其它后处理技术对肠道病变的诊断具有较高的临床价值。
Objective: To investigate the diagnostic value of 64-slice spiral CT virtual endoscopy (CTVE) and other post-processing techniques in the diagnosis of intestinal diseases. Methods: Totally 75 cases of suspected intestinal diseases were subjected to air-filled CT 64-slice enhancement scanning and reconstructed on ADW4.4 workstation, including CTVE, MPR, VR, VD and MIP images. The operative, pathological or follow-up results as a reference standard, the 64-slice CT diagnosis of intestinal lesions. Results: Of the 75 patients examined, 49 cases were classified as organic lesions by clinical diagnosis. 45 cases were detected by 64-slice CT, 5 cases were missed, one case was false positive, the sensitivity was 89.9% and the specificity was 96.2% The predicted value was 97.8%, the negative predictive value was 83.3% and the accuracy was 92%. CTVE for the mucosal surface lesions and morphological changes showed good, but need to be combined with other post-treatment methods to make the correct diagnosis. Conclusion: 64-slice CTVE and other post-processing techniques have high clinical value in the diagnosis of intestinal diseases.
