[摘要] 目的 探讨64层螺旋CT三维重建技术在四肢骨关节创伤中的临床应用价值。 方法 选取2010年8月~2014年1月共159例四肢骨关节外伤患者的三维成像影像学资料进行回顾性分析,均使用飞利浦64层螺旋CT扫描,行螺旋CT扫描后薄层重建,利用多平面重建(MPR)、曲面重建(CPR)、三维容积重建(VR)进行图像后处理进行观察。 结果 三维图像(MPR、CPR、VR图像)后处理技术对关节脱位、骨折、骨折合并脱位显示清晰,并能清晰显示骨折的部位、类型及骨折错位、碎骨片等空间关系;图像准确无误、具体、细微丰富,具有重要的临床应用价值。结论 几种重建方法的结合应用,参考CT的原始横断图像进行综合分析,不仅提高了对骨关节的诊断准确率,而且为临床预测手术可能性、制定手术方案、术式、评估手术效果提供了重要的临床应用价值。

Application of 64-slice spiral CT three-dimensional imaging in limb bone joint trauma

[Abstract] Objective To investigate the clinical application value of 64-slice spiral CT three-dimensional imaging techniques in the limb bone joint trauma. Methods The three-dimensional imaging data of 159 patients with limb bone joint trauma from August 2010 to January 2014 were selected and analyzed retrospectively. All the patients were given Philips 64-slice spiral CT scanning and thin slice reconstruction after the spiral CT scanning. Multi-planar reconstruction(MPR),curved planar reconstruction(CPR) and three-dimensional volume reconstruction(VR) were used for image postprocessing. Results Three-dimensional imaging(MPR,CPR and VR iamges) postprocessing techniques were able to show joint dislocation, fracture and fracture combined with dislocation clearly and were able to display the location and types of fractures and the spatial relationship such as fracture dislocation and bone chips. The images were accurate, specific and subtle, which were of important clinical application value. Conclusion It suggest that the combined application of several reconstruction methods and the comprehensive analysis referring to original traverse images of CT not only can improve the diagnosis accuracy rate for bone joint, but also provide important clinical application value for the clinical predication of surgical possibility, development of surgical plans, surgical methods and evaluation of surgical effects.

随着多层螺旋CT广泛应用于外伤性骨关节的检查,其价值已日益受到临床和影像学方面的重视[1]。其扫描速度快和容积数据采集,在此基础上进行三维成像,可直观、精确地显示正常解剖及病变的立体形态,得到不同组织类型的各自三维图像,可以详细了解各解剖结构的空间关系[2],从而直观地为临床医师提供更多的三维空间的信息,在骨关节创伤中的应用越来越显示出强大优势[3]。
1 资料与方法
1.1 一般资料
我科从2010年8月起开展此项技术,至2014年1月患者检查的四肢骨关节CT三维成像中,选取159例患者,男89例,女70例,年龄4~71岁,平均41.5岁。
