目的观察眼外伤及其复方樟柳碱治疗后眼供血系统血液动力学改变。方法用眼血流图(rheoophthalmography,ROG)和超声多普勒(Dopplerultrasonography,DUSG)检测34例眼外伤患者双眼睫状动脉(ciliaryartery,CA)、双侧眼动脉(ophthalmicartery,OA)、颈内动脉(internalcarotidartery,ICA)及颈总动脉(commoncarotidartery,CCA)的变化。结果发现伤眼和对侧健眼ROG波幅值均降低,OA收缩期血流速度增快,ICA、CCA收缩期最高峰值速度增高。提示伤后伤眼和对侧眼血流量减少,OA、ICA、CCA有不同程度痉挛、狭窄。表明外伤不仅直接影响伤眼的血管运动功能,而且创伤性刺激可通过同感反应,影响对侧眼、植物神经系统及皮层功能,使眼供血系统血液动力学异常。用复方樟柳碱治疗,可改善和恢复血管舒缩功能,缓解及消除血管痉挛,改善眼组织血供,从而提高视功能。结论ROG和DUSG联合检查,为外伤性眼供血系统血液动力学异常的诊治提供了客观依据。
Objective To observe the changes of hemodynamics of ocular blood supply system after ocular trauma and its compound anisodine treatment. Methods 34 eyes of ciliary artery (CA), ophthalmic artery (OA), internal carotid artery (ICA) of 34 patients with ocular trauma were examined with ROG and DUSG. (ICA) and common carotid artery (CCA). The results showed that the amplitude of ROG wave in injured eyes and contralateral healthy eyes decreased, the velocity of blood flow in OA systolic phase increased and the peak peak velocity of ICA and CCA systolic phase increased. Prompted injured eyes and contralateral eye blood flow decreased, OA, ICA, CCA have varying degrees of spasticity, stenosis. It shows that traumatic injury not only directly affects the vasomotor function of traumatic eyes, but also the traumatic stimulation can affect the contralateral eye, autonomic nervous system and cortical function by the same reaction and make hemodynamics abnormality of ocular blood supply system. Compound anisodine treatment can improve and restore vasomotor function, relieve and eliminate vasospasm, improve blood supply to the eye tissue, thereby enhancing visual function. Conclusion The combined examination of ROG and DUSG provides an objective basis for the diagnosis and treatment of hemodynamic abnormalities in traumatic ocular blood supply system.
