目的借助免疫组化技术,在光镜下确诊恶性间皮瘤。方法运用胰蛋白酶消化SP法及商品化的癌胚抗原(carcinoembryonicantigen,CEA)、角蛋白(keratin)和波形蛋白(vimentin),对30例恶性间皮瘤组织进行了免疫组化研究。结果(1)恶性间皮瘤组织基本不表达癌胚抗原,藉此可以剔除癌的混杂。(2)恶性间皮瘤具有既表达细胞角蛋白(阳性率43%),又表达波形蛋白(阳性率70%)的免疫组化特征,藉此可以与其它间叶来源的恶性肿瘤相区别。结论同时运用3种抗体进行免疫组化染色,可为在光镜下确诊恶性间皮瘤提供可靠的诊断依据。
Objective To identify malignant mesothelioma under light microscope by immunohistochemistry. METHODS: Immunohistochemical studies of 30 cases of malignant mesothelioma were performed using trypsin-digested SP and commercially available carcinoembryonic antigen (CEA), keratin, and vimentin. Results (1) Carcinoembryonic antigen was not expressed in malignant mesothelioma tissue, so that the mixed cancer could be eliminated. (2) Malignant mesothelioma has immunohistochemical features that express both cytokeratin (positive rate 43%) and vimentin (positive rate 70%), thereby distinguishing it from other mesenchymal tumors of the mesenchymal origin. Conclusion The simultaneous immunohistochemical staining of three kinds of antibodies can provide a reliable diagnosis basis for the diagnosis of malignant mesothelioma under light microscope.
