目的探究特发性血小板减少性紫癜(ITP)病患血清中可溶性Fas(s Fas)、可溶性Fas配体(s Fas L)、β2-微球蛋白(β2-MG)以及细胞因子的水平,并分析其相关性。方法选择2013年1月至2014年12月在海南省澄迈县人民医院接受治疗的ITP病患73例,并记为观察组。再随机选择同期在本院进行健康体检的志愿者73名,同时记为对照组。对比两组血清中s Fas、s Fas L、β2-MG及相关细胞因子水平,最后进行相关性分析。结果观察组患者s Fas、s Fas L、β2-MG、CD4+及CD28+水平分别为(14.94±2.08)、(0.67±0.13)、(2.63±0.45)、(25.61±4.28)及(32.74±7.03),显著高于对照组患者的(5.82±1.46)、(0.33±0.06)、(1.27±0.19)、(36.09±5.12)、(50.17±10.36),差异有统计学意义(P均<0.05);而观察组IL-2水平(0.31±0.07)低于对照组(0.62±0.18),差异有统计学意义(P<0.05)。相关性分析结果显示,s Fas、s Fas L及β2-MG与ITP呈正相关,而IL-2、CD4+及CD28+与ITP呈负相关,差异有统计学意义(P<0.05)。结论 s Fas、s Fas L和β2-MG参与了ITP的发病过程,可以应用于病变的诊断及分型。并且ITP病患免疫功能出现缺陷,对预后效果有一定的指导价值。
Objective To investigate the levels of soluble Fas (s Fas), soluble Fas ligand (s Fas L), β2-microglobulin (β2-MG) and cytokines in patients with idiopathic thrombocytopenic purpura (ITP) Analyze its relevance. Methods 73 patients with ITP who were treated in Chengmai People’s Hospital of Hainan Province from January 2013 to December 2014 were selected and treated as observation group. Randomly selected at the same period in our hospital for physical examination of 73 volunteers, at the same time as the control group. The levels of sFas, FasL, β2-MG and related cytokines were compared between the two groups, and finally the correlation analysis was carried out. Results The levels of Fas, FasL, β2-MG, CD4 + and CD28 + in the observation group were (14.94 ± 2.08), (0.67 ± 0.13), (2.63 ± 0.45), (25.61 ± 4.28) and (32.74 ± 7.03) (5.82 ± 1.46), (0.33 ± 0.06), (1.27 ± 0.19), (36.09 ± 5.12) and (50.17 ± 10.36) respectively, the difference was statistically significant (all P <0.05); The level of IL-2 in observation group (0.31 ± 0.07) was lower than that in control group (0.62 ± 0.18), the difference was statistically significant (P <0.05). Correlation analysis showed that s Fas, s Fas L and β2-MG were positively correlated with ITP, while IL-2, CD4 + and CD28 + were negatively correlated with ITP, the difference was statistically significant (P <0.05). Conclusion Fas, FasL and β2-MG are involved in the pathogenesis of ITP and can be used in the diagnosis and classification of lesions. And ITP patients with immune function defects, the prognostic effect of a certain guiding value.
