通过对97例流行性出血热患者的阴离子隙(AG)和血清 Na~+、K~+、Cl~-、HCO_3~-、白蛋白和钙的含量以及内生肌酐清除率变化的同步动态监测,结合临床资料分析,本研究发现,EHF 中 AG 降低的主要原因是低白蛋白血症和血液稀释所致的未测定阴离子减少.AG 降低的出现率依轻中型、重型、危重型而递减:病死者生前未见 AG 降低,且 AG 降低者无1例死亡;AG 降低时的内生肌酐清除率显著低于 AG 正常时,且显著高于 AG 升高时,维持在正常的50%左右.提示在 EHF 中 AG 降低的出现标志着病情较轻、肾小球滤过率中等程度下降,预后良好.
