目的:调查该院早产儿视网膜病变(ROP)的发生率及其相关因素,并针对国内现有的ROP筛查标准进行对照,评估其在县市级医院的实用性。方法:统计2006年1月~2010年6月该院新生儿重症监护病房的494例新生儿住院期间眼底筛查结果,并按该院的筛查标准与2004年卫生部制定的ROP筛查标准(体重<2 000 g,孕周<32周;高危因素早产儿可以到2 200 g、孕周<34周)、英国推荐的ROP筛查标准(体重≤1 500 g或孕周≤31周)和美国推荐的ROP筛查标准(体重≤1 500 g或孕周≤28周)对筛查结果进行比较。结果:在接受筛查的494例患儿(早产儿464例,足月儿30例)中仅有7例早产儿发生ROP,30例足月儿中无1例发生ROP。结论:该院ROP发生率明显低于其他地区及国外的报道,其原因可能与该院现有的筛查标准相对较宽及该地区经济水平相对偏低等有关,将该院的筛查结果与国内现有的ROP筛查标准比较无一例遗漏,与英国及美国的ROP筛查标准比较均有2例ROP患儿被遗漏,有必要根据更多的流行病学结果,制定出符合我国国情的ROP筛选体系。
Objective: To investigate the incidence and related factors of retinopathy of prematurity (ROP) in this hospital and to evaluate its practicability in county-level hospitals according to the existing ROP screening criteria in China. Methods: The fundus screening results of 494 newborns in neonatal intensive care unit of our hospital from January 2006 to June 2010 were collected. According to the screening standard of the hospital and the ROP screening standard established by the Ministry of Health in 2004 (Weight <2 000 g, gestational age <32 weeks; risk factors for preterm children up to 2 200 g, gestational age <34 weeks), the UK recommended ROP screening criteria (weight ≤ 1500 g or gestational age ≤ 31 weeks) And the United States recommended ROP screening criteria (weight ≤ 1500 g or gestational age ≤ 28 weeks) to compare the screening results. RESULTS: Of the 494 infants (464 preterm and 30 infants), ROP occurred in only 7 preterm infants and none in 30 infants with ROP. Conclusion: The incidence of ROP in this hospital is significantly lower than those reported in other regions and abroad. The reasons may be related to the relatively wide screening standard existing in the hospital and the relatively low economic level in the hospital. The screening results Compared with the existing ROP screening standard in our country, there is no omission. Two cases of ROP were missed compared with the ROP screening standards in Britain and the United States. It is necessary to draw out more epidemiological results according to China’s national conditions ROP screening system.
