目的提高处方合格率。方法将帕累托图用于不合格处方的原因分析工作中,找出影响处方合格率的“关键的少数和次要的多数”的关系。结果造成处方不合格的前3项因素(前记部分填写不全,药品规格、用法用量书写不完全或不规范,临床诊断或英文缩写不规范)为主要因素,字迹潦草难以辨认、处方书写完毕未划斜线、未采用药品通用名称为次要因素,其余为一般因素。结论针对造成本院不合格处方的主要原因,本院药事管理委员会应制订出台相应的奖惩措施条款,把影响不合格处方的主要因素及次要因素短时间内限制在可控范围内。
Objective To improve the passing rate of prescriptions. Methods Pareto charts were used in the analysis of the causes of unqualified prescriptions to find the relationship between the “key minorities and the minor majority” that affected the passing rate of prescriptions. As a result, the first three factors (pre-written partial incomplete, drug specification, usage and dosage were not complete or not standardized, clinical diagnosis or abbreviation not standardized) were the main factors that resulted in the unqualified prescriptions, the illegible characters were illegible, the prescription was not written Draw slash, did not use the generic name of the drug as a secondary factor, the rest of the general factors. Conclusion In view of the main reason for the unqualified prescription in our hospital, the Board of Pharmacy of the hospital should formulate the corresponding provisions of reward and punishment measures so as to limit the major and minor factors that affect the prescription of substandard prescriptions within the controllable range within a short period of time.
