目的观察右美托咪定对先心病患儿麻醉诱导期患者血流动力学的影响。方法 30例快通道麻醉先心病患儿随机分为2组。Dex组(n=15):患儿入室泵注右美托咪定1μg/(kg.h)15min。N组(n=15)作为对照组,以相同速度泵注生理盐水。随后6%七氟醚、0.1μg/kg顺阿曲库铵诱导行气管插管。记录收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)及心率(HR),以给药前(T0)、给药后3min(T1)、6min(T2)、9min(T3)、12min(T4)、15min(T5)、插管前(T6)、插管时(T7)作为观察记录点。结果 Dex组T5与T0比较,HR明显减慢(P﹤0.05)。麻醉诱导期间,N组T7与T6比较,SBP、DBP、MAP、HR值均有升高(P﹤0.05)。结论右美托咪定以1.0μg/(kg.h)剂量输注对简单房、室间隔缺患儿产生明显镇静效应,复合6%七氟醚麻醉诱导期间的血流动力学更加稳定,减轻插管反应。
Objective To observe the effect of dexmedetomidine on hemodynamics in patients with congestive heart failure during congestive heart failure. Methods Thirty patients with fast-track anesthesia congenital heart disease were randomly divided into two groups. Dex group (n = 15): children received intravenous injection of dexmedetomidine 1μg / (kg.h) 15min. N group (n = 15) as a control group, the same speed of saline injection. Tracheal intubation was then induced with 6% sevoflurane, 0.1 μg / kg cisatracurium. The systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP) and heart rate (HR) were recorded before treatment (T0) , 12min (T4), 15min (T5), intubation (T6), intubation (T7) as the observation record. Results Compared with T0, HR in Dex group was significantly decreased (P <0.05). During induction of anesthesia, SBP, DBP, MAP and HR increased in group T compared with group T6 (P <0.05). Conclusion Dexmedetomidine infusion of 1.0μg / (kg.h) had a significant sedative effect on children with simple ventricular septal defect and sevoflurane, and the haemodynamics during the induction of sevoflurane combined with 6% sevoflurane were more stable and relieved Intubation reaction.
