目的:了解妊娠期糖尿病(GDM)患者血清脂联素(APN)水平与糖脂代谢的关系,探讨APN是否参与GDM孕妇糖脂代谢紊乱的形成。方法:采用ELISA法测定30例妊娠期糖尿病孕妇和30例正常糖耐量孕妇(NGT)空腹血清APN水平;同时测定两组孕妇空腹血清总胆固醇(TCH)、甘油三酯(TG)、极低密度脂蛋白胆固醇(VLDL-C),空腹血糖(FPG)及糖化血红蛋白(HbA1c)水平。结果:①GDM组孕妇血清APN水平明显低于NGT组,差异有统计学意义(P<0.05)。②GDM组孕妇血TCH、TG、VLDL-C水平明显高于NGT组孕妇(P<0.05)。③GDM组孕妇FPG、HbA1c水平明显高于NGT组孕妇(P<0.01)。④相关分析显示,两组孕妇血清APN与孕晚期BMI、FPG、HbA1c、TCH、TG及VLDL-C均存在负相关关系(P<0.01或P<0.05),与孕前BMI无明显相关性(P>0.05)。结论:GDM患者血APN水平明显降低,APN水平降低可能参与诱导GDM患者糖脂代谢紊乱的形成。
Objective: To investigate the relationship between serum adiponectin (APN) level and glucose and lipid metabolism in patients with gestational diabetes mellitus (GDM) and to explore whether APN participates in the formation of glucose and lipid metabolism disorder in pregnant women with GDM. Methods: The levels of APN in 30 pregnant women with gestational diabetes mellitus and 30 normal glucose tolerance pregnant women (NGT) were measured by ELISA. The levels of fasting serum total cholesterol (TCH), triglyceride (TG), extremely low density Lipoprotein cholesterol (VLDL-C), fasting blood glucose (FPG) and glycosylated hemoglobin (HbA1c) levels. Results: ① The level of serum APN in GDM group was significantly lower than that in NGT group (P <0.05). ② The levels of TCH, TG and VLDL-C in GDM group were significantly higher than those in NGT group (P <0.05). The levels of FPG and HbA1c in pregnant women with GDM were significantly higher than those in NGT pregnant women (P <0.01). ④ Correlation analysis showed that there was a negative correlation between serum APN levels and BMI, FPG, HbA1c, TCH, TG and VLDL-C in the third trimester (P <0.01 or P <0.05) > 0.05). Conclusion: The level of blood APN in GDM patients is obviously decreased. The decreased APN level may be involved in the induction of glucose and lipid metabolism disorders in GDM patients.
