为了解绍兴地区男男性接触者(men who have sex with men,MSM)艾滋病病毒(human immunodeficiency virus,HIV)、梅毒感染状况,高危行为发生情况及影响因素,并为在该人群中制定更加有效的预防控制措施提供科学依据。本研究利用2011-2013年绍兴市MSM哨点监测数据,对MSM人群HIV、梅毒感染状况及高危行为进行动态分析。2011-2013年分别调查了252、427和414例MSM,3年间最近6个月同性肛交性行为发生率较高,波动于75.18%~85.27%;最近6个月发生同性肛交性行为时安全套坚持使用率较低,波动于38.83%~50.15%;最近6个月发生异性性行为安全套坚持使用率较低,波动于8.00%~21.21%。文化程度越高,最近6个月肛交性行为发生率越低;与女性在婚/同居、31~50岁年龄组MSM肛交性行为发生率高。2011-2013年绍兴市MSM人群HIV、梅毒感染率分别为7.14%、4.68%、2.90%和6.75%、6.55%、2.17%,各年度间差异均有统计学意义。绍兴地区MSM人群HIV、梅毒感染率近3年有下降趋势,但是该人群高危性行为较普遍,尤其中年人群肛交性行为发生率高,感染风险大,有必要提高干预检测的针对性和有效性,及早发现感染者,遏制艾滋病、梅毒在MSM人群中的传播。
To understand the status of HIV, syphilis infection, risk behaviors and influencing factors among men who have sex with men (MSM) in Shaoxing, and to find out more effective Prevention and control measures to provide a scientific basis. In this study, MSM sentinel surveillance data from 2011 to 2013 were used to analyze the status of HIV, syphilis infection and high-risk behaviors in MSM population. From 2011 to 2013, 252,427 and 414 MSM cases were investigated respectively. The prevalence of same-sex anal sex was high in the recent 6 months in 3 years, with a fluctuation ranging from 75.18% to 85.27%. Condom adherence to same-sex anal sex in the recent 6 months The rate of use was low, fluctuating between 38.83% and 50.15%. Condom use during the last 6 months was insignificant, fluctuating between 8.00% and 21.21%. The higher the educational level, the lower the incidence of anal sex in the past 6 months; the higher the incidence of MSM anal sex in women aged 31 to 50 who are married / living together. In 2011-2013, the infection rates of HIV and syphilis among MSM population in Shaoxing were 7.14%, 4.68%, 2.90% and 6.75%, 6.55% and 2.17%, respectively. The differences among the years were statistically significant. In the MSM population in Shaoxing, HIV and syphilis infection rates have been declining in recent 3 years. However, high-risk behaviors are common in this population. Especially in middle-aged people, the incidence of anal intercourse is high and the risk of infection is high. Therefore, it is necessary to improve the pertinence and effectiveness of intervention testing Sexually transmitted, early detection of infected people, containment of AIDS and syphilis in MSM populations.
