目的:观察精索静脉曲张(VC)患者精子形态学参数改变。方法:106例VC患者精液样本及对照组26例健康男性精液样本,均按严格标准,采用计算机辅助分析系统下人工修正的方法对精子形态进行分析。结果:VC组形态正常精子百分率显著低于对照组(P<0.001),头部缺陷精子百分率、含胞质小滴精子百分率均显著高于对照组(P<0.001,P<0.05);与对照组相比,VCⅡ度和Ⅲ度组形态正常精子百分率均显著降低(P<0.05,P<0.001),而对照组与VCⅠ度组形态正常精子百分率差异无显著性(P>0.05)。VCⅢ度组形态正常精子百分率显著低于Ⅱ度组(P<0.001);与VCⅠ度组相比,VCⅡ度、Ⅲ度组形态正常精子百分率均显著降低(P<0.05,P<0.001);VC组畸形精子指数和精子缺陷指数也显著高于对照组(P<0.001)。结论:VC可导致精子形态改变,使精子畸形程度加深。形态正常精子百分率随VC程度加深而降低。
Objective: To observe the morphological changes of sperm in patients with varicocele (VC). Methods: Totally 106 semen samples from VC patients and 26 healthy male semen samples from the control group were analyzed for their sperm morphology according to strict criteria and manually modified by computer-assisted analysis system. Results: The percentage of morphologically normal spermatozoa in VC group was significantly lower than that in control group (P <0.001). The percentage of defective sperm and the percentage of sperm containing sperm droplets in VC group were significantly higher than those in control group (P <0.001, P <0.05) (P <0.05, P <0.001). However, there was no significant difference in the percentage of morphologically normal sperm between control group and VC group (P> 0.05). The percentage of morphologically normal spermatozoa in VCⅢ group was significantly lower than that in the second degree group (P <0.001). Compared with VCⅠ group, the percentage of normal spermatozoa in VCⅡ, Ⅲdegree group were significantly decreased (P <0.05, P <0.001) Group abnormal sperm index and sperm defect index were also significantly higher than the control group (P <0.001). Conclusion: VC can lead to the change of sperm morphology and deepen the deformity of sperm. The percentage of morphologically normal sperm decreased with the degree of VC deepened.
