Nowadays, cases of multi-severed limbs are not rare. It is important to use the uninjured part to restore the injured extremity with cross transplantation. A case of severed right arm, right thigh and left leg in a train accident was admitted to our department. His right leg was transplanted onto the left thigh. Prolonged rehabilitation resulted in a satisfactory ability of weight bearing and daily activities 15 months after the accident. Recommendation for clinical management were put forward and literature reviewed.
