3例特发性舌咽神经痛的典型重症患者，经施行颌下进路舌咽神经颈段切除术后，彻底消除了疼痛发作。切下神经进行了光电镜检查，主要病理变化为神经纤维脱髓鞘改变。文中着重介绍了颌下进路手术方法，并结合舌咽神经的解剖及其特性，对手术治疗中通常采用的经颅内及颅外进路（口咽和预外）进行了讨论，认为颌下进路是安全、简单、值得推荐的方法。
