目的:探讨异基因外周血干细胞移植(allo-pbsct)治疗晚期恶性淋巴瘤的疗效。方法:患者诊断为淋巴肉瘤白血病,预处理方案为大剂量环磷酰胺(CTX)加全身照射(TBI),供者为HLA相同的胞妹,用粒细胞集落刺激因子(G~CSF)8ug·kg~(-1)·7.25~(-1)连续5.5天对供者进行外周血干细胞动员,于第5天及第6正(即相当于移植的一1及0天)用Cobespectra血液细胞分离机采集单个核细胞(MNC),两次采集的MNC悬液均于0天由静脉输给患者。移植后给予预防移植物抗宿主病(GVHD)等措施。结果:患者迅速恢复造血功能,于+13天WBC>1.0×10~9/L,TBC>20×10~9/L(不输血)以后逐渐恢复正常,+14天骨髓增生活跃,性染色体核型转变为供者型(46,XX),现已随访至90天,骨髓仍持续缓解。结论:allo—PBSCT是治疗晚期恶性淋巴瘤的有效手段。
Objective: To investigate the efficacy of allogeneic peripheral blood stem cell transplantation (allo-pbsct) in the treatment of advanced malignant lymphoma. Methods: The patient was diagnosed as lymphosarcoma leukemia. The pretreatment regimen was high-dose cyclophosphamide (CTX) combined with whole body irradiation (TBI). The donor was a sibling sister with HLA and granulocyte colony-stimulating factor (G-CSF) 8 ug. Kg~(-1)·7.25~(-1) continuous peripheral blood stem cell mobilization for donors for 5.5 consecutive days, and isolation on Cobespectra blood cells on days 5 and 6 (equivalent to days 1 and 0 of transplantation) Mononuclear cells (MNCs) were harvested and the two collected MNC suspensions were intravenously delivered to patients on day 0. After transplantation, prevention of graft-versus-host disease (GVHD) and other measures are given. Results: The patient quickly recovered hematopoietic function, gradually returned to normal after +13 days WBC>1.0×10~9/L, TBC>20×10~9/L (without blood transfusion), +14 days of bone marrow hyperplasia, sex chromosome nucleus The type changed to a donor type (46, XX), which has now been followed up to 90 days and the bone marrow continues to remit. Conclusion: Allo-PBSCT is an effective method to treat advanced malignant lymphoma.
