目的考察宫内感染新生儿和中重度缺氧缺血性脑病(HIE)新生儿的不良指标对比。方法选择2010年5月~2014年1月在本院产科分娩的88例新生儿,其中经病理检查诊断的宫内感染的新生儿45例,中重度HIE新生儿43例。结果两个组别无死亡案例。两个组的性别比、5 min Apgar、低血糖、高血K+和CT检查无显著差异(P>0.05),与宫内感染组比,HIE组母亲孕期感染、不良孕产史、出生低体重和胎膜早破例数较少(P<0.05),妊娠期高血压、胎位异常、低血Ca2+和Na+例数较多(P<0.05),血清和脑脊液肌酸磷酸激酶同工酶(CK-BB)和神经烯醇化酶(NSE)水平较高(P<0.01)。结论宫内感染新生儿和中重度HIE新生儿均存在神经系统受损情况,但受损程度和预后不同,可通过比较母亲孕期感染、不良孕产史、出生低体重、胎膜早破、妊娠期高血压疾病、胎位异常、低血Ca2+和Na+,血清和脑脊液CK-BB和NSE来区分诊断。
Objective To compare the adverse outcomes of neonates with intrauterine infection and neonates with moderate and severe hypoxic-ischemic encephalopathy (HIE). Methods From May 2010 to January 2014, 88 newborns with obstetric delivery in our hospital were selected. Among them, 45 were neonates with intrauterine infection diagnosed by pathology and 43 were neonates with moderate-severe HIE. Results There were no deaths in both groups. There were no significant differences in sex ratio, 5 min Apgar, hypoglycemia, hyperkalemia, K + and CT in the two groups (P> 0.05). Compared with the intrauterine infection group, the HIE group had lower rates of pregnancy during pregnancy, And premature rupture of membranes (P <0.05). There were more cases of gestational hypertension, abnormal fetal position and low blood Ca2 + and Na + (P <0.05), serum and CSF creatine phosphokinase isoenzyme (CK- BB) and neuroenoenolase (NSE) levels were higher (P <0.01). Conclusions Neonatal intrauterine infection and moderate to severe neonatal hypothyroidism have nervous system damage, but the extent of the damage and prognosis may be different through the mother’s pregnancy infection, adverse pregnancy history, low birth weight, premature rupture of membranes, pregnancy Hypertensive disease, fetal position abnormalities, low blood Ca2 + and Na +, serum and cerebrospinal fluid CK-BB and NSE to distinguish the diagnosis.
