目的:探讨凋亡相关基因survivin在非小细胞肺癌中表达及其与bcl-2的关系。方法:选取52例非小细胞肺癌患者组织,其中鳞癌26例、腺癌24例、大细胞未分化癌2例。正常肺组织10例,分别应用免疫组织化学法检测survivin及bcl-2蛋白的表达。结果:survivin表达率在正常肺组织为0,非小细胞肺癌为65.39%,2组比较差异有统计学意义;在高分化癌组织为44.44%,在中-低分化癌组织为76.47%,2组比较差异有统计学意义;在有淋巴结转移组为82.61%,无淋巴结转移组为51.72%,2组比较差异有统计学意义。Survivin的表达与患者年龄、性别、吸烟史及病理分型无关。survivin在bcl-2阳性表达组和阴性表达组中的表达率分别为90.00%和37.93%,差异有统计学意义。结论:survivin基因异常表达引起的细胞凋亡抑制在非小细胞肺癌发生中起重要作用,其过度表达提示非小细胞肺癌预后不良;survivin蛋白表达与bc1-2蛋白表达呈正相关,二者在非小细胞肺癌发生中可能起协同作用。联合检测survivin蛋白和bc1-2蛋白,可为判断非小细胞肺癌侵袭、转移的能力、预后提供依据。
Objective: To investigate the expression of survivin in non-small cell lung cancer and its relationship with bcl-2. Methods: Totally 52 cases of non-small cell lung cancer tissues were selected, including 26 cases of squamous cell carcinoma, 24 cases of adenocarcinoma and 2 cases of large cell undifferentiated carcinoma. Normal lung tissue 10 cases, immunohistochemistry were used to detect the expression of survivin and bcl-2 protein. Results: The expression of survivin was 0 in normal lung tissue and 65.39% in non-small cell lung cancer. The difference between the two groups was statistically significant. The positive rate of survivin was 44.44% in well differentiated carcinoma, 76.47% in moderately differentiated carcinoma, There was significant difference between the two groups (82.61% in lymph node metastasis group and 51.72% in non-lymph node metastasis group). The difference between the two groups was statistically significant. The expression of Survivin was not related to patient’s age, sex, smoking history and pathological type. The expression rates of survivin in bcl-2 positive group and negative group were 90.00% and 37.93% respectively, the difference was statistically significant. Conclusion: The inhibition of apoptosis induced by the abnormal expression of survivin gene plays an important role in the development of non-small cell lung cancer. The overexpression of survivin gene suggests that the prognosis of non-small cell lung cancer is poor. The expression of survivin protein is positively correlated with the expression of bcl- Small cell lung cancer may play a synergistic role. Combined detection of survivin protein and bc1-2 protein may provide basis for judging the invasion and metastasis of non-small cell lung cancer and prognosis.
