目的应用二维超声斑点追踪成像(STI)技术检测高血压患者左室心肌内、中、外层组织应变特征,探讨心肌中层应变在高血压患者左室收缩功能评价中的价值,为早期诊断及治疗高血压心肌病变提供依据。方法 78例原发性高血压患者,根据左室肥厚诊断标准分为A组(无左室肥厚组,41例)、B组(左室肥厚组,37例),30例健康志愿者作为对照组。取左室短轴乳头肌水平切面,将室壁平均分为三层,即内膜层、中间心肌层、外膜层。获取各组各层心肌圆周应变(SSC)峰值。结果各组心肌收缩期峰值应变内膜层>中间心肌层>外膜层(P<0.05)。A组中间心肌层SSC峰值较对照组增高(P<0.05),B组的内膜层较A组及对照组减低(P<0.05)。结论心肌中层应变对高血压患者早期左室收缩功能异常的诊断有着重要的临床意义。
Objective To evaluate the value of myocardial mid-strain in the assessment of left ventricular systolic function in hypertensive patients by two-dimensional ultrasonography with spot-tracking imaging (STI) Treatment of hypertensive cardiomyopathy provide basis. Methods 78 patients with essential hypertension were divided into group A (without left ventricular hypertrophy group, 41 cases), group B (left ventricular hypertrophy group, 37 cases) and 30 healthy volunteers group. Take left ventricular short axis papillary muscle horizontal section, the wall is divided into three layers, namely the endometrium, the middle myocardium, outer membrane. The myocardial SSC peak value of each group was obtained. Results The peak systolic myocardial endocardium> middle myocardial layer> epicardium in each group (P <0.05). The peak value of SSC in middle myocardium of group A was higher than that of control group (P <0.05), while that of group B was lower than that of group A and control group (P <0.05). Conclusion Myocardial strain in the midst of hypertensive patients with early diagnosis of left ventricular systolic dysfunction has an important clinical significance.
