目的:探讨减影3D—CTA成像技术对颅内动脉瘤的诊断作用。方法:对36例临床蛛网膜下腔出血的病人行CT检查,重建技术用最大密度投影(MIP)和容积再现(VR)。以手术及数字减影血管造影术(digital subtraction angiography,DSA)为标准评价减影3D—CTA和3D—CTA检查结果。结果:减影3D—CTA检出动脉瘤45个,敏感度为100%,特异度为100%,阴性似然比为0.0000:3D—CTA检出动脉瘤43个,漏检2个颈内动脉床突下的动脉瘤,敏感度为97.8%,特异度为100%,阴性似然比为0.0220。结论:减影3D—CTA技术对颈内动脉床突下动脉瘤的检出具有明显的优势。
Objective: To investigate the diagnostic value of subtraction 3D-CTA imaging for intracranial aneurysms. Methods: Thirty-six patients with clinical subarachnoid hemorrhage underwent CT examination. Reconstruction techniques were performed using maximum density projection (MIP) and volume rendering (VR). Subtraction 3D-CTA and 3D-CTA results were evaluated by surgery and digital subtraction angiography (DSA). Results: 45 aneurysms were detected by subtraction 3D-CTA with 100% sensitivity, 100% specificity and negative likelihood ratio of 0.0000. Forty-three aneurysms were detected by 3D-CTA, and two internal carotid arteries were missed Subcortical aneurysms, the sensitivity was 97.8%, specificity was 100%, negative likelihood ratio of 0.0220. Conclusion: Subtractive 3D-CTA technique has obvious advantages for the detection of subclavian artery aneurysm.
