目的:研究袋形术对牙源性角化囊性瘤(KCOT)患区牙的影响,为患区牙的保存治疗提供依据。方法:将42例患者分为两组,A组行一期袋形术+二期刮除术,B组行单纯刮除术。术后每3个月复诊1次,术前及复诊时分别检查并记录受累牙的临床状况(松动度及牙髓活力)。结果:42例KCOT病变内受累牙共计237颗。A组患者的受累牙总保存率为92.66%,与B组的91.4%相比无明显统计学差异,其中Ⅰ-Ⅱ°松动受累牙保存率分别为:A组(100%),B组(78.57%)。A组的受累牙牙髓活力保存率71.56%,明显高于B组的36.72%(P<0.01)。结论:一期袋形术+二期刮除术相比单纯刮除术而言,可较多恢复受累牙的稳固、保存牙髓活力,有利于长期发挥正常咀嚼功能。
Objective: To study the effect of orthopedic surgery on teeth in patients with odontogenic keratocystic neoplasia (KCOT) and to provide basis for the treatment of teeth in the affected area. Methods: Forty-two patients were divided into two groups. Group A was performed with one stage suturing and two stage curettage while group B with simple curettage. A follow-up was conducted every 3 months after surgery. The clinical status (degree of loosening and pulp vitality) of the affected teeth were examined and recorded before and during the follow-up. Results: A total of 237 affected teeth were found in 42 cases of KCOT lesions. The total dental preservation rate of group A was 92.66%, which was not significantly different from that of group B (91.4%). The preservation rates of group Ⅰ-Ⅱ ° loosened teeth were: group A (100%), group B 78.57%). The dental pulp vitality retention rate of group A was 71.56%, which was significantly higher than that of group B (36.72%, P <0.01). Conclusions: Compared with simple curettage, the one-stage suture + two-stage curettage can restore the stability of the affected tooth more and preserve the viability of the pulp, which is good for long-term normal masticatory function.
