作者报告了近3年多来治疗的46例松果体区肿瘤,认为脑 CT 检查对于术前肿瘤定性有一定帮助。这些病例皆采用枕部经小脑幕入路,作到肿瘤全切除或大部分切除者占80.43%,手术死亡率为4.34%。由于此部位肿瘤直按手术死亡率近10余年来已明显降低,而肿瘤约有1/4组织学为良性,故作者主张治疗的首选方案为直接切除肿瘤。经过多年来各种手术入路的比较,作者认为用枕部经小脑幕上入路有较大的优越性。
The authors report 46 cases of pineal region tumors treated for more than three years and concluded that brain CT examination may be of some help in the preoperative tumor characterization. These cases are the use of occipital transocclusion of the occipital approach, as the total tumor resection or most of the excision accounted for 80.43%, surgical mortality was 4.34%. Since this part of the tumor direct surgery mortality rate has been significantly reduced over the past 10 years, while about 1/4 of the tumor histology is benign, so the author claims that the preferred treatment for the treatment of direct tumor removal. After many years of various surgical approaches, the author believes that the use of the occipital transapical transapical approach has greater superiority.
