目的 探讨消化道癌组织中P 糖蛋白 (P gp)、热休克蛋白 (HSP2 7)和拓扑异构酶 (TopoⅡ )表达及意义。方法 收集 16 3例消化道癌标本 ,包括食道癌 4 3例 ,胃癌 4 8例 ,大肠癌 72例 ,采用石蜡切片和免疫组织化学SP法染色。结果P gp在消化道癌中的阳性率为 4 9 6 9% ,HSP2 7在消化道癌中的阳性率为 5 7 6 7% ,TopoⅡ阳性率为 4 7 85 %。P gp和HSP2 7在高分化组阳性率高于低分化组 (P <0 0 5 ) ,而TopoⅡ消化道癌低分化组阳性率高于高分化组 (P <0 0 5 )。TopoⅡ在食道癌组阳性表达率高于胃癌 (P <0 0 5 ) ;而P gp在大肠癌中的表达率明显高于胃癌 (P <0 0 5 )。结论三种耐药标志物与癌组织分化程度相关 ;联合检测消化道癌组织中P gp、HSP2 7和TopoⅡ ,对制定化疗方案有指导作用
Objective To investigate the expression and significance of P gp, HSP27 and Topo Ⅱ in gastrointestinal cancer. Methods Totally 163 cases of specimens of gastrointestinal cancer were collected, including 43 cases of esophageal cancer, 48 cases of gastric cancer and 72 cases of colorectal cancer. Paraffin sections and immunohistochemical SP staining were used. Results The positive rate of Pgp in digestive tract cancer was 49.69%. The positive rate of HSP27 in digestive tract cancer was 57.67%. The positive rate of Topo II was 47.75%. The positive rates of Pgp and HSP27 in well-differentiated group were higher than those in poorly-differentiated group (P <0.05), while those in poorly differentiated group were higher than those in well-differentiated group (P <0.05). The positive rate of Topo Ⅱ in esophageal cancer was higher than that in gastric cancer (P <0.05), while the expression of P gp in colorectal cancer was significantly higher than that in gastric cancer (P <0.05). Conclusions The three drug resistance markers are correlated with the degree of differentiation of cancer tissues. Combined detection of P gp, HSP2 7 and Topo Ⅱ in gastrointestinal carcinomas can guide the development of chemotherapy regimens
