目的探讨了垂体动态增强扫描对垂体微腺瘤诊断价值。方法选择2013年1月至2014年1月间我院疑诊为垂体微腺瘤患者88例为研究对象,所有患者均于入院后2d内行鞍区MR平扫、动态增强扫描及延迟扫描。以手术病理及临床证实结果为标准诊断,观察并比较MR平扫、动态增强扫描及延迟扫描对垂体微腺瘤诊断情况。结果经病理或临床证实为垂体微腺瘤56例,均为单发,其中位于垂体前叶49例,左侧27例,右侧29例。长径2~10mm,平均5.3±2.2mm。MR平扫显示垂体内异常信号28例,动态增强扫描显示病灶55例,延迟增强扫描显示病灶32例。动态增强扫描对垂体微腺瘤显示率显著高于平扫及延迟增强,差异具有统计学意义(P<0.05)。动态增强扫描对垂体微腺瘤诊断敏感性、特异性、阳性预测率及阴性预测率分别为94.6%、93.7%、96.4%、90.9%。结论垂体动态增强扫描对垂体微腺瘤诊断价值高,值得临床推广应用。
Objective To investigate the diagnostic value of dynamic contrast-enhanced pituitary gland micro-adenoma. Methods From January 2013 to January 2014, 88 cases of suspected pituitary adenoma in our hospital were selected as study subjects. All patients underwent MR scan, dynamic contrast-enhanced scan and delayed scan within 2 days after admission. The pathological and clinical confirmed results as the standard diagnosis, observation and comparison of MR plain scan, dynamic contrast-enhanced and delayed scan diagnosis of pituitary adenoma. Results The pathological or clinical confirmed 56 cases of pituitary microadenomas were single, which was located in 49 cases of anterior pituitary, left in 27 cases, right in 29 cases. Long diameter 2 ~ 10mm, an average of 5.3 ± 2.2mm. MR plain scan showed abnormal signal in pituitary 28 cases, dynamic enhanced scan showed 55 cases of lesions, delayed enhancement scan showed lesions in 32 cases. Dynamic enhanced scan of pituitary adenoma showed significantly higher than plain scan and delayed enhancement, the difference was statistically significant (P <0.05). The sensitivity, specificity, positive predictive rate and negative predictive value of dynamic contrast-enhanced scanning in diagnosing pituitary microadenomas were 94.6%, 93.7%, 96.4% and 90.9%, respectively. Conclusion The dynamic contrast-enhanced pituitary gland diagnosis of pituitary adenoma has high value, which is worthy of clinical application.
