目的探讨射频消融(RFA)对兔肝VX2移植瘤细胞凋亡的影响。方法以种植瘤组织悬液法成功建立20只兔VX2肝癌模型,移植瘤直径达(2.5±0.5)cm后随机均分为四组:A组直接处死取肝脏标本;B、C、D组分别于RFA后4、12及24h处死取出肝肿瘤组织。HE染色观察组织病理结构改变,dUTP缺口末端标记测定(TUNEL)法检测肿瘤凋亡,电镜下观察凋亡细胞的形态学表现。结果 VX2肿瘤呈浸润性生长。B、C、D组见肿瘤中央射频消融区呈现大片坏死,残癌细胞皱缩,染色质浓聚边集,凋亡小体形成,周围见炎性反应带,外围癌组织残留。A组肿瘤细胞凋亡指数为0.14±0.04;B、C、D组肿瘤细胞凋亡指数分别为0.47±0.03、0.68±0.04和0.49±0.05,均明显高于A组(P<0.05)。结论 RFA治疗能消融局部肿瘤,促进坏死周围残存肿瘤凋亡,遏制肿瘤活性。
Objective To investigate the effect of radiofrequency ablation (RFA) on the apoptosis of VX2 transplanted liver in rabbits. Methods Twenty tumor models of VX2 hepatocellular carcinoma were established successfully by implanting tumor tissue suspension. The diameter of the tumor was (2.5 ± 0.5) cm and then divided into four groups randomly. A group was directly sacrificed to take liver samples; B, C and D groups At 4, 12 and 24h after RFA, the liver tumor tissues were removed. The histopathological changes were observed by HE staining. The apoptotic cells were detected by dUTP nick end labeling (TUNEL) assay. The morphological changes of apoptotic cells were observed under electron microscope. Results VX2 tumors showed invasive growth. In group B, C and D, large necrosis was observed in the radiofrequency ablation area of the tumor, and the residual tumor cells were shrunk, the chromatin condensed edge set and the apoptotic body were formed, and the surrounding inflammatory reaction zone and the peripheral cancer tissues remained. The apoptosis index of tumor cells in group A was 0.14 ± 0.04. The apoptosis index of group B, C and D were 0.47 ± 0.03,0.68 ± 0.04 and 0.49 ± 0.05 respectively, which were significantly higher than those in group A (P <0.05). Conclusion RFA can ablate local tumors, promote apoptosis of residual tumor around necrosis, and inhibit tumor activity.
