支气管哮喘患儿血清25-羟基维生素D3水平变化及其与血清IgE和FeNO的相关性

[目的]探讨支气管哮喘患儿血清25-羟基维生素D3[25(O H)D3]水平变化及其与血清免疫球蛋白E (IgE)和呼出气一氧化氮(FeNO)的相关性.[方法]选取支气管哮喘患儿108例,按随机数表法分为观察组和对照组,每组54例.另外选取同期儿保健康体检儿童50例(健康组).对照组患儿给予常规治疗,观察组患儿在常规治疗基础上给予维生素D治疗3个月,比较两组疗效.同时比较受试者血清25(O H)D3水平、总IgE水平、Fe-NO,并分析血清25(O H)D3水平与血清总IgE和FeNO相关性.[结果]观察组治疗总有效率为90.74%(49/54),高于对照组的75.93%(41/54),其差异有统计学意义(P <0.05);观察组、对照组患儿血清25(O H)D3水平显著低于健康组(P <0.05),血清总IgE水平、FeNO均显著高于健康组(P <0.05).Pearson相关分析显示:血清25(O H)D3水平与IgE和FeNO呈负相关(r = -0.825,P = 0.001;r = -0.876,P =0.001).[结论]补充维生素D辅助治疗可改善小儿支气管哮喘疗效;患儿血清25(O H)D3水平与气道高反应性和机体变应反应指标(血清总IgE和FeNO)呈负相关.“,”[Objective] To explore the efficacy of vitamin D adjuvant therapy in children with bronchial asthma and the relationship between the concentration of serum 25-hydroxyvitamin D3[25(OH)D3]and the indexes(IgE and FeNO)of airway hyperresponsiveness and organism allergic response indexes.[Methods]A total of 108 chil-dren with bronchial asthma were selected for the study and randomly divided into the observation group(n =54)and the control group(n =54).In addition,50 healthy children during the same period were selected as healthy group.The control group was given routine treatment for asthma,and the observation group was given vitamin D intervention for 3 months on the basis of routine treatment.The efficacy of the two groups was com-pared.At the same time,the serum levels of vitamin D3,serum total IgE and exhaled NO(FeNO)were recor-ded among groups,and the relationship between serum 25(OH)D3level and serum total IgE and FeNO was ana-lyzed.[Results] The total effective rate of treatment in the observation group was 90.74%(49/54),which was higher than that in the control group 75.93%(41/54);the difference was statistically significant(P <0.05). The levels of serum vitamin D3in both control and observation groups were significantly lower than that in healthy group(P < 0.05),while the total serum levels of IgE and FeNO were higher than those in healthy group(P <0.05).Pearson correlation analysis showed that serum 25(OH)D3level was negatively correlated with IgE and FeNO(r = -0.825,P = 0.001;r = -0.876,P =0.001).[Conclusion] Vitamin D supplementa-tion adjuvant therapy can enhance the efficacy of pediatric bronchial asthma and improve pulmonary function.Se-rum 25(OH)D3level is negatively correlated with airway hyperresponsiveness and body allergic response indexes (serum total IgE and FeNO).
