目的探讨128层螺旋CT血管成像在颈脑血管疾病中的诊断价值。方法收集2009年3月至2010年9月,我院临床怀疑颈脑血管疾病981例患者,应用128层螺旋CT进行颈脑血管成像检查,回顾性分析颈脑血管疾病的各种影像学征象。结果颅内动脉瘤89例,颅内动静脉畸形25例,颈脑血管狭窄524例(颈总动脉狭窄53处,颈内动脉起始段狭窄183处,烟雾病3例,椎动脉起始段狭窄73处,左侧锁骨下动脉起始段狭窄75处,颅内动脉狭窄195处),颅内乙状窦血栓3例,海绵窦瘘3例,血管变异78例,无异常发现259例,阳性检查率73.59%(722/981)。结论 128层螺旋CT血管成像诊断颈脑血管疾病,具有高效、无创、诊断准确等优点,对评价其风险,制定治疗计划有重要的临床应用价值。
Objective To investigate the value of 128-slice spiral CT angiography in the diagnosis of cervical and cerebrovascular diseases. Methods From March 2009 to September 2010, 981 patients with suspected cervical and cerebrovascular diseases in our hospital were collected. Cervical and cerebrovascular imaging was performed with 128-slice spiral CT. Retrospective analysis of various imaging signs of cervical and cerebrovascular diseases. Results 89 cases of intracranial aneurysms, 25 cases of intracranial arteriovenous malformations, 524 cases of cervical and cerebral vascular stenosis (common carotid artery stenosis 53, the initial section of internal carotid stenosis 183, moyamoya disease in 3 cases, the initial section of vertebral artery Stenosis of 73, the beginning of the left subclavian artery stenosis 75, intracranial arterial stenosis 195), intracranial sigmoid sinus thrombus in 3 cases, cavernous sinus fistula in 3 cases, vascular abnormalities in 78 cases, no abnormal findings in 259 cases, The positive rate was 73.59% (722/981). Conclusion 128-slice spiral CT angiography can diagnose cervical and cerebrovascular diseases, which has the advantages of high efficiency, non-invasive and accurate diagnosis. It has important clinical value in evaluating the risk and making treatment plan.
