目的:探讨血降钙素原(PCT)和脑脊液血管内皮细胞生长因子(VEGF)水平变化在早期新生儿细菌性脑膜炎(BM)中的意义。方法:细菌性脑膜炎组36例实验组入院初及恢复期取脑脊液应用酶联免疫吸附法测定VEGF,采静脉血应用电化学发光法测定PCT。对照组30例一次性测定脑脊液VEGF及血PCT。结果:细菌性脑膜炎组脑脊液VEGF水平为(144.21±98.08)pg/m L,明显高于对照组的(71.64±12.88)pg/m L(P<0.01);细菌性脑膜炎组血PCT水平为(5.26±9.27)μg/L,明显高于对照组的(0.27±0.13)μg/L(P<0.01);以脑脊液VEGF≥85.56 pg/L、血PCT≥0.46μg/L为阳性标准,二项指标对提示细菌性脑炎的敏感度分别为80.6%、97.2%,特异度为90%、96.7%。结论:脑脊液VEGF、血P CT水平变化可作为早期诊断新生儿BM,判断病情及预后的重要指标。
Objective: To investigate the significance of the changes of blood procalcitonin (PCT) and cerebrospinal fluid vascular endothelial growth factor (VEGF) level in early neonatal bacterial meningitis (BM). Methods: 36 cases of bacterial meningitis group at the beginning of admission and convalescent cerebrospinal fluid were collected for the determination of VEGF by enzyme-linked immunosorbent assay. The venous blood samples were collected for determination of PCT by electrochemiluminescence. Control group 30 cases of a one-time determination of cerebrospinal fluid VEGF and blood PCT. Results: The level of VEGF in cerebrospinal fluid of patients with bacterial meningitis was (144.21 ± 98.08) pg / m L, significantly higher than that of the control group (71.64 ± 12.88) pg / m L (P <0.01) (5.26 ± 9.27) μg / L, which was significantly higher than that of the control group (0.27 ± 0.13) μg / L (P <0.01). The positive rates of CSF≥85.56 pg / L and PCT≥0.46 μg / The sensitivity of the two indexes to prompt bacterial encephalitis were 80.6% and 97.2% respectively, and the specificity was 90% and 96.7% respectively. Conclusion: The changes of VEGF and blood CT level in cerebrospinal fluid can be used as an important index for early diagnosis of neonatal BM and prognosis.
