衔接因子蛋白含pH域蛋白1(APPL1)可作为细胞内衔接蛋白介导胰岛素信号通路、脂联素信号通路等多种信号转导,并且还可参与炎症反应、胰岛素敏感性、胰岛素抵抗、糖尿病肾病( DKD)相关的血流动力学、肾脏纤维化病变等,所以 APPL1可能参与了 DKD 的病理生理反应。另外,胰岛素抵抗与DKD密切相关,高胰岛素血症可通过直接损伤肾脏细胞,可导致体内多种炎性因子的增加。因此,阐明APPL1与胰岛素抵抗及DKD的关系,将为DKD的治疗提供新思路。“,”Adaptor protein, phosphotyrosine interaction, PH domain and leucine zipper containing 1 (APPL1)can be used as an intracellular adaptor protein mediating a variety of signal transduction such as insulin signaling pathway and adiponectin signaling pathway ,which can also participate in hemodynamic and renal fibrosis pathological changes related with inflammation ,insulin sensitivity ,insulin resistance and diabetic kidney disease( DKD) ,so it may participate in the pathological physiological reaction of DKD .In addition, insulin resistance is closely associated with DKD and hyperinsulinemia can increase variety of inflammatory factors in the body by direct damaging the kidney cells.Therefore,the elucidation on the relation between APPL1 and insulin resistance as well as DKD,will provide new ideas for the treatment of DKD.
