目的:探讨磁共振扩散加权成像(MR DWI)在高级别胶质瘤和单发脑转移瘤中的鉴别诊断价值。方法9例单发脑转移瘤和10例高级别胶质瘤均进行了常规磁共振成像(MRI)及扩散加权成像(DWI),在经后处理重建获得的表观扩散系数(ADC)图分别测得其肿瘤实质部分及瘤周水肿区的ADC值,并进行对比分析。结果(1)9例单发脑转移瘤均表现为边界清晰的类圆形病灶,瘤周水肿带范围较广。10例高级别胶质瘤表现为不规则斑片状(n=6)或类圆形(n=4),边界清(n=4)或不清(n=6),瘤周水肿带不如转移瘤明显。(2)单发脑转移瘤和高级别胶质瘤肿瘤实质部分ADC值分别为(1.11±0.19)×10-3mm2/s和(1.43±0.33)×10-3mm2/s,两者比较差异无统计学意义(t=2.15,P=0.05)。单发脑转移瘤和高级别胶质瘤瘤周水肿区ADC值分别为(1.69±0.22)×10-3mm2/s和(1.24±0.18)×10-3mm2/s,两者比较差异有统计学意义(t=4.45,P<0.01)。结论 MRI DWI序列显示高级别胶质瘤和单发脑转移瘤瘤周水肿区的ADC值差异有统计学意义,对两者的鉴别诊断有一定帮助。“,”Objective To study the value of MRI diffusion weighted imaging in differential diagnosis between high grade gliomas and single brain metastases.Methods 9 cases of single brain metastases and 10 cases of high grade gliomas underwent conventional MRI and DWI imaging. The ADC value of parenchyma and peritumoral edema region in each case were measured and analyzed.Results (1)All 9 cases of single brain metastases showed round lesions with a wide range of peritumoral edema. 10 cases of high grade gliomas showed irregular patchy (n=6) or round (n=4) mass and peritumoral edema zone. (2)The ADC value of parenchyma in single brain metastases and high grade gliomas were (1.11±0.19)×10-3mm2/s and (1.43±0.33)×10-3mm2/s, respectively. There was no signifi cant difference between them (t=2.15,P=0.05). The ADC value of peritumoral edema region in single brain metastases and high grade gliomas were (1.69±0.22)×10-3mm2/s and (1.24±0.18)×10-3mm2/s respectively. There was signifi cant difference between them (t=4.45,P<0.01).ConclusionMRI DWI sequences revealed signifi cant difference in ADC value of peritumoral edema region between high grade gliomas and single brain metastases, which is helpful in the differential diagnosis.
