【目的】探讨早期康复治疗对脑瘫高危儿转归的疗效影响。【方法】通过病例对照研究,将218例年龄1~8个月的脑瘫高危儿分成两组,其中118例进行早期康复治疗为康复组,100例因各种原因不能参加康复治疗患儿为常规育儿组。研究开始两组在性别、神经系统查体、Gesell发育测评方面差异无统计学意义(P>0.05)。其中康复组要保证住院接受综合康复治疗10~15 d,对主要监护人进行康复技术培训后继续家庭康复,并定期随访接受康复指导。观察6个月时两组再次进行Gesell发育测评,1岁时确诊或排除脑性瘫痪,并进行脑瘫分度测评。【结果】观察6个月后康复组与常规组患儿Gesell发育测评组间比较差异有统计学意义(P<0.01);满1岁时康复组118例中发生脑瘫9例,发生率7.6%,常规组100例中发生脑瘫32例,发生率32%,脑瘫的发生率组间比较差异有统计学意义(P<0.01)。【结论】早期康复治疗可促进脑瘫高危儿全面发育,降低脑瘫发生率,减轻脑瘫的程度。
【Objective】 To investigate the effect of early rehabilitation on the outcomes of high-risk infants with cerebral palsy. 【Methods】 A total of 218 high risk infants with cerebral palsy who were 1 to 8 months old were divided into two groups according to the case control study. Among them, 118 were treated early and recovered, and 100 were children who could not participate in rehabilitation because of various reasons Parenting group. There was no significant difference in gender, neurological examination and Gesell development between the two groups (P> 0.05). Among them, the rehabilitation group should ensure that hospitalized recipients receive comprehensive rehabilitation 10 to 15 days, continue the family rehabilitation after the rehabilitation training for the main guardians, and receive regular rehabilitation follow-up instructions. After 6 months, the two groups were again evaluated for Gesell development, diagnosed or excluded cerebral palsy at the age of 1, and assessed for cerebral palsy sub-degree. 【Results】 The results showed that there was significant difference between Gesell developmental assessment group in rehabilitation group and routine group after 6 months (P <0.01), and 9 cases of cerebral palsy occurred in 118 cases of rehabilitation group at the age of 1, the incidence rate was 7.6% In the conventional group, 32 cases of cerebral palsy occurred in 100 cases, the incidence rate was 32%. The incidence of cerebral palsy was significantly different between the two groups (P <0.01). 【Conclusion】 Early rehabilitation can promote the development of high risk infants with cerebral palsy, reduce the incidence of cerebral palsy and reduce the degree of cerebral palsy.
