本文介绍1例眶底部皮样囊肿采取改良的眶外侧进路得以顺利摘除。患者:刘××女24岁汉族1982年3月20日因左眼球突出七个月入院。患者于1981年7月发现左眼比右眼略大,溢泪。3个月后左眼突出较明显,视力逐渐减退,经消炎治疗无效而来我院。检查:左眼视力0.4,眼眶部皮肤无水肿,未触及肿块,结膜和角膜正常。眼底检查:左眼视乳头充血,边缘模糊,生理凹陷消失,静脉略粗,A:V=3:5,乳头周边未见出血或渗出,黄斑部略暗。眼球向正前方突出,眼球突出计检查:右眼12mm,左眼18mm,眶距100mm眼球活动不受限,俯卧后眼球突出度不增加。A型超声波检查发现左眼球后外上方有实质性肿块。X—线平片检查:双侧眼眶等大,未见骨质缺损,双侧视神经孔等大,左侧眼眶未见骨凹窝形成。眶静脉造影未成功,脑血管造影见左眼动脉中外1/3处迂曲抬高,副鼻窦X—线摄片无特殊所见。初步诊断:左眼眶占位性病变。
This article describes a case of orbital dermoid cyst with modified orbital approach to successfully removed. Patients: Liu XX female 24-year-old Han March 20, 1982 due to left eye protrusion seven months admitted to hospital. Patients in July 1981 found that the left eye slightly larger than the right eye, overflowing with tears. 3 months after the left eye prominent more obvious vision gradually diminished by anti-inflammatory treatment ineffective to our hospital. Check: left eye vision 0.4, orbital skin edema, untreated mass, conjunctival and corneal normal. Fundus examination: left eye papilla congestion, fuzzy edge, disappearance of physiological depression, vein slightly thick, A: V = 3: 5, no bleeding or oozing around the nipple, macular slightly darker. Eye prominent in front of the eye prominent meter check: right eye 12mm, left eye 18mm, orbital distance 100mm eye activity is not limited, do not increase the projection of the eye after prone position. A-type ultrasound examination found that the left eye after the top of a substantial mass. X-ray plain film examination: bilateral orbital and other large, no bone defects, bilateral optic nerve hole and other large, left orbital bone recess was not found. Orbital venography was unsuccessful, cerebrovascular angiography seen in the left eye artery 1/3 at tortuous elevation, paranasal sinus X-ray film no special view. Preliminary diagnosis: left orbital space-occupying lesions.
