目的:探讨头颈肿瘤与外科治疗对患者生存质量的影响。方法:本次共选择50例头颈肿瘤患者作研究对象,均为我院2012年9月~2013年9月收治,分析不同病变类型及外科治疗方式对生存质量的影响。结果:舌癌PSS~HN平均分明显低于鼻咽癌、喉癌(P<0.05),但3种不同部分头颈肿瘤在PACT-H&N平均分比较上,无明显差异(P>0.05)。术前PSS-HN评分267.7分,术后1个月为164.7分,有明显下降(P<0.05)。术前FACT-H&N为114.4分,术后1个月为92.6分,有统计学意义(P<0.05)。术后6个月,喉部分切除术PACT-H&N评分高于全切术,舌癌直接拉拢缝合高于舌癌前臂游离皮瓣修复,有统计学差异(P<0.05)。结论:依据头颈肿瘤不同部位、类型,对手术和修复方案进行个体化选择,可显著提高生存质量。 Objective: To investigate the impact of head and neck cancer and surgical treatment on the quality of life of patients. Methods: A total of 50 patients with head and neck cancer were selected as research objects. All of them were treated in our hospital from September 2012 to September 2013 and analyzed the effect of different types of lesions and surgical treatment on quality of life. Results: The average score of PSS ~ HN in tongue cancer was significantly lower than that in nasopharyngeal carcinoma and laryngeal cancer (P <0.05). However, there was no significant difference in PACT-H & N average score between three different head and neck tumors (P> 0.05). Preoperative PSS-HN score of 267.7 points, one month after surgery was 164.7 points, decreased significantly (P <0.05). Preoperative FACT-H & N was 114.4 points, one month after surgery was 92.6 points, with statistical significance (P <0.05). At 6 months after operation, the PACT-H & N score of laryngectomy was higher than that of total resection. The direct suture of tongue cancer was higher than the free flap of tongue cancer forearm repair, with statistical difference (P <0.05). Conclusion: According to the different parts and types of head and neck cancer, the individualized selection of surgery and rehabilitation programs can significantly improve the quality of life.
