目的:探讨肝硬化Child-Pugh A级患者预防性经内镜食道曲张静脉套扎(EVL)对门静脉血流量和肝功能的影响及其相关性。方法:对16例Child-Pugh A级患者食道曲张静脉行预防性EVL,套扎前后采用彩色多普勒超声对门静脉血流参数定量测定,并结合肝功能指标的变化进行分析。结果:门静脉血流量由套扎前的994.86±175.03mL/min升高至套扎后三个月的1070.45±217.61mL/min和六个月的1074.93±224.09mL/min,患者血清白蛋白浓度、总胆固醇浓度以及血浆凝血酶原时间均较套扎前显著改善;门静脉血流量的增加同上述肝功能指标的改善,呈显著直线相关。结论:彩色多普勒超声可以准确评价EVL术前后门静脉血流参数的变化,预防性EVL可以通过增加门静脉血流改善Child-Pugh A级患者的肝功能。
Objective: To investigate the effects of prophylactic endoscopic esophageal varicose vein ligation (EVL) on portal vein blood flow and liver function in Child-Pugh class A patients with cirrhosis and its correlation. Methods: Preventative EVL was performed in 16 cases of Child-Pugh class A patients with esophageal varices. Quantitative determination of portal vein blood flow parameters by color Doppler echocardiography was performed before and after ligation, and the changes of liver function indexes were analyzed. Results: The portal venous flow increased from 994.86 ± 175.03mL / min before ligation to 1070.45 ± 217.61mL / min for three months after ligation and 1074.93 ± 224.09mL / min for six months. The serum albumin concentration, Total cholesterol and plasma prothrombin time were significantly improved than those before ligation; portal vein blood flow increased with the improvement of liver function was significantly linear correlation. Conclusion: Color Doppler ultrasound can accurately evaluate the change of portal vein blood flow parameters before and after EVL. Prophylactic EVL can improve liver function in Child-Pugh class A patients by increasing portal vein blood flow.
