目的:探讨妊娠合并卵巢肿瘤及瘤样病变患者的临床诊断方法及治疗对策,为卵巢肿瘤的治疗提供理论依据。方法:选取2012年1月~2013年9月该院收治的妊娠合并卵巢肿瘤及瘤样病变患者42例。对其临床资料进行分析,比较常规妇科检查与B超检查阳性检出率的差异;并比较不同孕周孕妇B超检查阳性率、术后妊娠情况、不同肿瘤直径患者并发症发生率以及妊娠情况。结果:常规妇科检查检出妊娠合并卵巢肿瘤及瘤样病变患者13例(30.95%),B超检查检出38例(90.47%),B超检查明显优于常规妇科检查,差异有统计学意义(P<0.05)。<20孕周孕妇B超检查阳性率(100.00%)和术后正常妊娠率(100.00%)均明显高于其他孕周,差异有统计学意义(P<0.05)。肿瘤直径≥5 cm的患者并发症发生率(64.28%)高于肿瘤直径<5 cm的患者(3.57%),两者比较,差异有统计学意义(P<0.05)。结论:孕妇于16~20孕周进行B超检查,妊娠合并卵巢肿瘤及瘤样病变阳性检出率最高,手术成功率高,术后妊娠情况好,流产死胎率低。B超检查有利于妊娠合并卵巢肿瘤及瘤样病变患者的临床诊断,及早进行手术治疗能保证孕妇术后正常妊娠。
Objective: To investigate the clinical diagnosis and treatment of pregnancy complicated with ovarian tumors and tumor-like lesions, and to provide a theoretical basis for the treatment of ovarian tumors. Methods: Forty-two patients with pregnancy complicated with ovarian tumors and tumor-like lesions admitted to our hospital from January 2012 to September 2013 were selected. The clinical data were analyzed to compare the difference between the positive detection rate of routine gynecological examination and B-ultrasound. The positive rate of B-ultrasound, gestational pregnancy, the incidence of complications in different tumor diameters and the pregnancy were compared . Results: 13 cases (30.95%) were diagnosed by routine gynecological examination in pregnancy with ovarian tumor and tumor-like lesions, 38 cases (90.47%) were diagnosed by B-mode ultrasound, B-ultrasound was better than routine gynecological examination, the difference was statistically significant (P <0.05). The positive rate of B-ultrasound (100.00%) and postoperative normal pregnancy (100.00%) in pregnant women <20 gestational weeks were significantly higher than those of other gestational weeks (P <0.05). The incidence of complications (64.28%) in patients with tumor diameter> 5 cm was higher than that in patients with tumor diameter <5 cm (3.57%), the difference was statistically significant (P <0.05). Conclusion: Pregnant women underwent B-ultrasound at 16 to 20 gestational weeks. Pregnancy with ovarian tumors and tumor-like lesions has the highest positive detection rate, high operation success rate, good postoperative pregnancy, and low rate of stillbirth. B-ultrasound is conducive to pregnancy with ovarian cancer and tumor-like lesions in patients with clinical diagnosis, early surgical treatment can ensure that pregnant women after normal pregnancy.
