目的探讨江苏省苏州、无锡、常州地区不同性别成人血尿酸(SUA)水平与代谢综合征(MS)的关系,为有效预防和控制MS提供依据。方法采用分层整群随机抽样的方法,于2009年3-11月在苏州、无锡、常州地区抽取年龄≥20岁的常住成人3 723例进行问卷调查、体格检查及相关代谢指标测定。结果男性MS和高尿酸血症的患病率(分别为21.54%、26.95%)均明显高于女性(分别为18.31%、11.15%),差异均有统计学意义(χ2值分别为5.869和140.403,P<0.05,P<0.01)。男性和女性MS检出率随着SUA水平的升高呈上升趋势,有统计学意义(趋势χ2值分别为103.229和99.419,P<0.01)。随着MS组分数目的增加,男性和女性SUA水平均随之增加,有统计学意义(r值分别为0.267和0.311,P<0.01)。logistic多元回归分析结果显示,在调整年龄、性别、吸烟、饮酒、文化程度等变量后,以SUA水平第1分位为参照,男性SUA水平第4分位发生MS的OR值为2.630(95%CI:1.849~3.740);女性SUA水平第4分位发生MS的OR值为2.955(95%CI:1.946~4.488)。结论无论男性还是女性,SUA水平与MS的发生均密切相关。在积极关注MS人群防控的同时,也应加强对SUA水平的监测和高尿酸血症的预防。
Objective To investigate the relationship between serum uric acid (SUA) levels and metabolic syndrome (MS) in adults of different genders in Suzhou, Wuxi and Changzhou, Jiangsu Province, and to provide evidence for effective prevention and control of MS. Methods A stratified cluster random sampling method was used to collect 3 723 permanent residents aged 20 years and older from March to November in 2009 in Suzhou, Wuxi and Changzhou to conduct questionnaire survey, physical examination and related metabolic indexes. Results The prevalence rates of male MS and hyperuricemia were significantly higher than those of women (21.54% and 26.95% respectively) (18.31% and 11.15% respectively) (χ2 = 5.869 and 140.403, respectively) , P <0.05, P <0.01). The detection rate of MS in males and females showed an upward trend with the rise of SUA levels (trend χ2 = 103.229 and 99.419, respectively, P <0.01). With the increase of the number of MS components, SUA levels in both men and women increased with statistical significance (r = 0.267 and 0.311, P <0.01). Logistic multiple regression analysis showed that the odds ratio (OR) of MS for MSA with SUA level was 2.630 (95%) after adjusting for age, gender, smoking, alcohol consumption and education level, CI: 1.849-3.740). The odds ratio of MS for female SUA level 4 was 2.955 (95% CI: 1.946-4.488). Conclusions Both male and female SUA levels are closely related to the occurrence of MS. In the active attention of MS crowd prevention and control should also strengthen the monitoring of SUA levels and prevention of hyperuricemia.
