目的了解肝癌高发区隆安县乙型肝炎病毒(HBV)前S基因缺失突变株的流行情况。方法HBV表面抗原(HBsAg)无症状携带者从我们隆安县研究队列中选择,用套式聚合酶链反应对研究对象血清HBV前S基因扩增和序列分析。结果在66例标本中9例出现前S基因缺失突变,占13.6%(9/66)。9例缺失突变均为框内突变,其中7例突变发生在或涉及前S2区的5’末端。男6例、女3例出现前S基因缺失突变,但男女间突变率(12.0%,6/50与18.8%,3/16)差异无统计学意义(χ2=0.709,P>0.10);前S基因缺失突变率在基因型B、基因型C和重组基因型之间差异无统计学意义(χ2=0.002,P>0.95)。结论肝癌高发区隆安县居民HBV无症状携带者前S基因缺失突变率较高,这种突变与肝癌的关系值得进一步探讨。
Objective To understand the prevalence of hepatitis B virus (HBV) pre-S gene deletion in Longan County, a high-risk area for hepatocellular carcinoma. Methods Asymptomatic carriers of HBV surface antigen (HBsAg) were selected from our Longan County study cohort and the pre-HBV HBV S gene was amplified and sequenced by nested polymerase chain reaction. Results In the 66 specimens, 9 cases had a deletion of the pre-S gene, accounting for 13.6% (9/66). All nine deletion mutations were in-frame mutations, of which seven mutations occurred at or involved the 5 ’end of the pre-S2 region. There was no significant difference in the mutation rate of S gene between male and female (12.0%, 6/50 and 18.8%, 3/16) in 6 males and 3 females (χ2 = 0.709, P> 0.10) The mutation rate of S gene was not significantly different between genotype B, genotype C and recombinant genotype (χ2 = 0.002, P> 0.95). Conclusions The mutation rate of the pre-S gene deletion in HBV asymptomatic carriers of residents in Long’an County with high incidence of HCC is high. The relationship between this mutation and HCC deserves further exploration.
