目的在心脏手术中应用血液管理,以有效减少异体血液的输注,节约血液资源。方法本院2013-2014年4 663例年龄>16周岁成人心脏体外循环下开胸手术如二尖瓣置换术,双瓣置换,主动脉瓣置换,房间隔缺损修补,主动脉根部重建、多瓣膜手术、胸主动脉手术等,其中2013年2 276例,2014年2 387例,男性2 398例,女性2 265例,年龄<50周岁的2 492例,大于50周岁的2 171例。结果 2014年相比2013年患者的输血费用减少,有极其显著性统计学差异,女性相比男性患者在输血费用,住院费用和住院时间均低,且有极其显著性差异。<50岁年龄组相比>50岁患者在输血费用,住院费用和住院时间均低,且有极其显著性差异。结论血液管理在心脏手术的应用是有效和可行的,可有效减少成人心脏手术异体血液的输注。
Objective To apply blood management in cardiac surgery to effectively reduce the infusion of allogeneic blood and save blood resources. Methods A total of 4663 adult patients aged> 16 years undergoing cardiopulmonary bypass during 2013 to 2014 underwent open heart surgery such as mitral valve replacement, double valve replacement, aortic valve replacement, atrial septal defect repair, aortic root reconstruction, Surgery and thoracic aortic surgery included 2 276 cases in 2013, 2 387 cases in 2014, 2 398 cases in males and 2 265 cases in females. There were 2 492 cases <50 years old and 2 171 cases over 50 years old. Results Compared with 2013, the blood transfusion cost decreased in 2014 compared with 2013, and there was a statistically significant difference between the two groups. Female transfusion costs, hospitalization costs and hospital stay were all lower than those of male patients in 2014, with extremely significant differences. Blood transfusion costs, hospitalization costs, and length of stay were lower in patients <50 years of age compared to> 50 years of age and had very significant differences. Conclusion The application of blood management in cardiac surgery is effective and feasible, which can effectively reduce the blood transfusion in adult cardiac surgery.
